Payroll Forms

• W-4 Employee Withholding Allowance Certificate:  The W-4 is required of each employee to determine the appropriate federal income tax deduction to be withheld from the employee’s pay each pay period. The filing status and the number of exemptions declared remain in effect unless and until a new W-4 is submitted by the employee, and will supersede any previous W-4(s).
